Metoprolol tartrate
Indications/Uses
High BP eg, arterial HTN; CHD eg, angina pectoris; functional CV symptoms eg, hyperkinetic heart syndrome; rapid forms of cardiac irregularity eg, tachycardiac arrhythmia. Acute & long-term therapy during & after MI. Prophylactic migraine therapy.
Dosage/Direction for Use
High BP, CHD 1 tab once daily. Functional angiocardiopathy ½ to 1 tab once daily. Rapid forms of CV irregularities, prophylactic migraine therapy 1 tab once daily or bid. Acute therapy after MI Initially 5 mg IV, & further 5 mg single dose at 2-min interval up to total dose of 15 mg, then 25-50 mg oral every 6 hr over 48 hr starting 15 min after last dose inj. Long-term therapy after MI Maintenance dose: ½ to 1 tab bid in 2 single dose.
Administration
Should be taken with food: Take after meal. Swallow whole w/ some liqd, do not chew.
Contraindications
Hypersensitivity. Stimulus conduction disorders from the atria to the ventricles (2nd & 3rd degree AV block); sick sinus syndrome; disordered stimulus conduction between sinus nodes & atria (high grade SA block); shock; myocardial insufficiency; bradycardia; hypotension; acidosis; late stages of peripheral circulatory disturbance; bronchial hyperreactivity. Concurrent use of MAOIs except MAO-B inhibitors.
Special Precautions
Severe anaphylactic shock. Diabetic patient w/ heavily fluctuating blood sugar values & in strict fasting. Patient w/ pheochromocytoma; restricted hepatic function. Personal or family history of psoriasis. Latent DM or worsened existing condition. May mask symptoms of lowered blood sugar eg, rapid pulse. Avoid abrupt discontinuation. Avoid alcohol intake. May affect the ability to drive & use machines. Pregnancy & lactation.
Adverse Reactions
Fatigue, dizziness, depressive moods, lightheadedness, light headache, sweating, sleep disturbances, increased dream activity, hallucinations. Nausea, vomiting, abdominal pains, constipation, diarrhea; dyspnoea; tingling sensation, feeling of coldness in extremities.
Drug Interactions
Intensified effects of insulin or oral hypoglycemics; antihypertensives. Stronger drop in BP w/ nitroglycerine. Fall in BP, bradycardia & other cardiac irregularities may occur w/ verapamil or other antiarrhythmics. Lowered antihypertensive reactions w/ adrenaline or other substances w/ sympathomimetic action eg, contained in cough mixtures, nasal & eye drops. Stronger drop in cardiac frequency or delay in stimulus conduction in heart w/ reserpine, α-methyldopa, clonidine, guanfacine or cardiac glycosides. Lowered plasma conc w/ rifampicin. Increased plasma conc w/ cimetidine. Reduced hypotensive effect w/ indomethacin. Reduced elimination of lidocaine. Reciprocally intensified sedative effects w/ alcohol. Reduced force of myocardial contraction w/ anaesth.
